NEW STUDENT REGISTRATION INFORMATION
Applications for the upcoming school year are now being accepted. Students must meet the academic and behavior criteria for admission to be accepted. Students seeking admission to PreKindergarten Three, Four and Kindergarten must be three (3), four (4) and five (5) by September 1 of the academic year.
PreK3 - 8th Grade
Registration Fee - $300
Registration Requirements:
• TWO YEARS of STANDARDIZED TEST SCORES
(CTBS, FCAT, SAT, IOWA, etc.).
• A copy of the previous year’s REPORT CARD and the first semester of the current academic year.
• A copy of your child/ren’s original BIRTH CERTIFICATE. (A copy will be made in the office and the original returned.)
• IF BAPTIZED CATHOLIC, a BAPTISM and FIRST COMMUNION certificate. (A copy will be made in the office and the original returned.)
• Social Security Card
• IMMUNIZATIONS: Check with your physician to be sure your child’s immunizations are up to date, prior to final registration. A new form is needed anytime a new immunization is given to keep our files up to date. The files are checked yearly and a report is required by the Health Department.
FLORIDA RESIDENTS: Students entering St. Paul’s Catholic School for the first time must present a Student Physical Examination form HRS #3040 (original, no copies) signed by a Florida physician and a current copy of Immunization form HRS #680 (original, no copies) returned to the office prior to final registration. These forms
are obtained at the physician’s office.
STUDENTS TRANSFERRING FROM OUT OF STATE: The Public Health Department will transfer the medical information to a Florida Certificate at no charge.
New students entering grades K - 8 may be given an assessment test.
New students must meet the academic and behavioral criteria for acceptance.
Students that meet the acceptance criteria are accepted for admission to the school regardless of race, sex, color, or country of origin.

WITHOUT REQUIRED REGISTRATION FEE.
